2026 ORIGINAL SIX PACK CHALLENGE REGISTRATION, WAIVER, AND LIABILITY RELEASE FORM
Please read and agree to all terms of this waiver and liability release form before signing.
In consideration of being permitted to participate in the 2026 Original Six Pack Challenge (OSPC)—including all related ceremonies and events—and to enter premises, facilities, or grounds supervised or controlled by the OSPC, Todd Watkins (TW), Jeff Dzikowicz, Raymond Cook, or any volunteer organizer of this event, I hereby release, discharge, and agree not to sue:
The Original Six Pack Challenge (OSPC), Todd Watkins (TW), the Manitoba Ball Hockey Association (MBHA), Westridge Community Centre (WCC), All Time Group Ltd. (ATG), Raymond Cook, Sean Hart, and their respective directors, members, trustees, agents, representatives, officers, sponsors, licensors, servants, volunteers, and employees.
I agree to hold them harmless and indemnify them from any and all liabilities, claims, injuries, losses, damages, expenses, demands, or actions of any kind— including death—arising out of or related to my participation in the 2026 Original Six Pack Challenge, whether caused by negligence or otherwise.
I acknowledge that all participants are considered part of the organizing committee, and that the MBHA, All Time Group Ltd., Ray Cook, Todd Watkins, Jeffrey Dzikowicz, and Westridge Community Centre are not partners in, nor responsible for, the organization of this event. Their involvement is strictly limited to providing advertising support for the 2026 Original Six Pack Challenge.
I agree to release and discharge the OSPC, TW, RC, MBHA, ATG, WCC, and their representatives from any and all claims, demands, or obligations that may arise from any injury or damage caused by me or to me while participating in this event or while on the premises.
I understand and accept the risks and hazards associated with sponge hockey, including but not limited to: slippery surfaces, obstructions, and the potential for injury or death. I confirm that I am a competent player and that I voluntarily assume all risks of loss, damage, or injury that may occur during participation in the 2026 Original Six Pack Challenge or while on the premises.
I agree to wear a CSA-approved helmet and unaltered, undamaged shoes designed for sponge hockey or broomball at all times while participating. It is strongly recommended that all players wear additional protective equipment suitable for the sport.
I agree not to enter or participate on the playing surface while under the influence of alcohol or drugs. I understand that there are no formal means of testing for such substances and accept full responsibility for monitoring my own condition. I agree to leave the ice immediately if asked by on-ice officials or designated representatives who suspect impairment.

I am 18 years of age or older, or, if under 18, my parent or guardian has co-signed below.
